1. Overview

Illumident Port Fairy is pleased to offer our valued patients the opportunity to participate in our Patient Referral Program. This initiative aims to thank current patients for referring new patients to our Port Fairy practice. This offer is not currently available at our Mount Gambier practice.

By participating in this program, you agree to the following Terms and Conditions:

2. Eligibility

  • The referrer must be an existing and active patient of Illumident Port Fairy at the time of the referral. Existing patients are those who have attended Illumident Port Fairy since the practice opened.
  • The new patient must be a first-time patient to Illumident Port Fairy and must attend and complete their first appointment for the referral to be eligible.
  • The new patient must list the full name of the referrer on their new patient form under the section “How did you hear about us?” at the time of their first visit or advise the Front Office Coordinator on duty of the patient’s name. This cannot be added or amended after the appointment has occurred.

3. Referral Credit

  • For each eligible referral, the referrer will receive a $50 credit toward their next dental treatment at Illumident Port Fairy.
  • The credit will be applied to the referrer’s account only after the new patient has attended and completed their first appointment.
  • If the new patient cancels, fails to attend, or does not complete their first appointment, the credit will not be applied.
  • Referral credits are not transferable, not redeemable for cash, and must be used by the referrer for their own treatment. Referral credits must be applied to treatment fees and may not be used for deposits or product purchases within the practice (e.g. Piksters, POLA whitening products, deposit for a crown).
  • If multiple successful referrals are made by an existing patient, the existing patient’s credit may accumulate. However, a maximum of only one $50 credit can be applied per appointment.

4. Patient Privacy

  • In accordance with privacy laws and our internal policies, we cannot confirm whether a person listed as a referrer or referee is a current or past patient of the practice.
  • If a referrer enquires about the status of their own credit, we will only advise if a credit has been applied to their account – we cannot confirm which patient referral it relates to, or whether a named individual is now attending the practice.

5. Program Availability & Modifications

  • This offer is available for a limited time and may be suspended or withdrawn at any time at the discretion of Illumident Port Fairy.
  • Illumident Port Fairy reserves the right to modify these Terms and Conditions without prior notice.
  • Any abuse of the program, including falsifying referrals or attempting to circumvent the program rules, may result in removal from the program and reversal of any awarded credits.
  • This offer cannot be applied retroactively (i.e. a referrer may not receive a credit if the new patient had their first appointment before this offer commenced).
